This is purely a guess, but I suspect someone meant that a typist had
omitted a paragraph and nobody noticed until it was too late.
The report I read said that the para had fallen off the bottom of the
page. I took this to mean they were using a wordprocessor, probably
DOS (WordPervert, perhaps, or some fringe software) rather than
Windoze, incorrectly set up for the paper size.
In order for the para to `fall off the page', the actual paper would
have to be longer than the size defined in the software setup, so
perhaps they were using it set for A4 but actually had 10x8 paper in
the printer (10x8 or `quarto' was the old business standard before A4,
still popular for private correspondance among the upper class).
