I have spent several hours trying to get pulldown lists and listboxes to
appear in forms.

I have no problems with displaying the following input types in forms:

Checkboxes by using:
<input type="checkbox" name="PDF" value="PDF">PDF</input>
<input type="checkbox" name="Word" value="Word">Word</input>
<input type="checkbox" name="PS" value="PS">PS</input>

Radio buttons by using:
<input type="radio" name="priority" value="Urgent">Urgent</input>
<input type="radio" name="priority" value="High">High</input>
<input type="radio" name="priority" value="Medium">Medium</input>
<input type="radio" name="priority" value="Low">Low</input>

Textboxes by using:
<input type="text" name="name" size="30"></input>

However, I am having many problems with displaying both pulldown lists and
listboxes, which I need to create for several different types of units (eg.
date lists, lists of individuals, etc).

One of my colleagues (unreachable this week) created the following section
in the XSL sheet:

name="value"><xsl:value-of
select="/page/doc_title"/></xsl:attribute></input>

         <tr><td align="right"><b>requester: </b></td><td align="left">
            <select name="requester">
               <option>Jack</option>
               <option>Paul</option>
               <option>Mark</option>
            </select></td></tr>
         <tr><td align="right"><b>General Comments: </b></td><td
align="left"><input name="general_comment" type="text" size="70"/></td></tr>
      </table> <br/>
      <xsl:apply-templates/>
   </form>
</xsl:template>

This is the only kind of select list that I can produce. Whenever I use
<select> or <input type = "select"> tags for creating pulldown or listboxes
for dates or other lists of names of other groups , it automatically appears
at the doc title level. I understand that the template entry above in the
XSL sheet is to have the same template always appear in the same place.
Yet, for the pages that I am now creating, I need to create a variety of
different selection lists, and they do not always appear in the same place
in the document.

Here is an example of what I try to use:

            <select name="Month" size="6">
                <option>January</option>
                <option>February</option>
                <option>March</option>
                <option>April</option>
                <option>May</option>
                <option>June</option>
                <option>July</option>
                <option>August</option>
                <option>September</option>
                <option>October</option>
                <option>November</option>
                <option>December</option>
            </select>

There are no processing errors. The listbox simply does not appear at all.

Even deleting the above-mentioned template section from the XSL sheet does
not allow me to display any of my customized select lists or pulldown lists.

What I want to do is create date groups, and other lists of names of people
that can be used anywhere in the document, not just at the doc title level
where the XSL template pulldown example is always applied.

Does anyone have any idea why I cannot get the basic pulldown lists or list
boxes to appear, and/or what I can do to make them appear?
