I'd be interested to hear (offlist) what your problems were - we produce a book of drug monographs from a fairly complex DTD (180 elements) in FrameMaker+SGML. It can be hard to set up, but it
works a treat when it's finished.
You could try XMetal as well, though the typographic control is not quite as precise. Other than that, you might look at something like FOP (http://xml.apache.org/fop/) or PassiveTex
(http://users.ox.ac.uk/~rahtz/passivetex/). Personally, I'd figure out the manual, but then we sell the software...:-)
