This is perfectly fine. Elements can be as recursive as you wish.
<!ELEMENT A (A?)>
is fine as a DTD.

Yes. The only thing XML requires is that you define parameter entities
before you use them. Beyond that order is insignificant, except when
you repeat declarations.
