It's not obvious :). Section 16 of the XSLT spec covers it, hidden very
well as an attribute of the top-level <xsl:output> element. Basically you
do this (in your case):
<xsl:output method="xml" doctype-system="foo.dtd" />
There's a lot of other useful stuff in the <xsl:output> element, too.
