I find the explanation that 

title provides the general context of which the current page is a part and
header1 provides the context/heading for the particular page

as a non confusing (at least to those i've had to teach), usable explanation.

The example:

> > <TITLE>Sample Questionnaire</TITLE>
> > <H1>Sample Questionnaire</H1>

is a degenerate case (where the two contexts are equivalent)

IMHO a better example could be:

<TITLE>Sample Questionnaire's</TITLE>
<H1>A Questionnaire for Reporting HTML (in)adequacies</H1>

and another page in the set could be:

<TITLE>Sample Questionnaire's</TITLE>
<H1>A Questionnaire for Reporting Computer failures</H1>

and another....
